Term Membership Application

General

1. All applicants are reminded to read the application form carefully before completing it. All parts of this application form must be duly completed and submitted together. Otherwise, the application will not be processed.
2. Only individuals aged 21 and above who are non-Singapore Citizens or non-Singapore Permanent Residents are eligible to apply for Term Membership.
3. Applications for Term Member must be supported by a proposer and a seconder. Only Ordinary, Life, Associate and Term members may propose or second an applicant for Term Membership.
4. The particulars of an applicant for Term Membership shall be displayed on the Club’s LCD for a period of not less than 2 weeks for the attention of members.
5. All applications are subject to approval by the Management Committee.
6. A rejected applicant may not submit another application until the expiry of 12 months from the date of rejection.
7. Applicants are not permitted to use the Club facilities until membership cards are issued to them.

• Digital photo(s) of yourself, your spouse, and child(ren) aged 12 to 20
• Employment pass(s), dependent pass(s) or passport(s) of yourself, your spouse, and child(ren) aged 12 to 20
• Marriage certificate/Divorce document
• Child(ren)’s birth certificate(s)
• Car log card/ insurance cover note, and I.U. number
• Employment letter

Car Label

1. First car label is free, the second and third car label is chargeable at $54.50 (one-time) and $218.00 (per annum) respectively.
2. Registration of the vehicle must be accompanied by a copy of the Vehicle Registration Card or Insurance Policy. The vehicle should be registered under the name of the Principal Member or Spouse.
3. If the vehicle is registered under a third party's name, a written consent must be provided from the third party to allow the member to drive the said vehicle to the Club's premises, and to indemnify the Club from any loss or damage.
4. Car label(s) issued are not transferable.

• I hereby declare that the particulars furnished in this application are true and correct.
• I am not an undischarged bankrupt and have not been convicted of any criminal offence in Singapore or abroad.
• I agree to my particulars being displayed on the Club’s Notice Board for the specified duration in accordance with the Club’s admission procedure.
• I acknowledge Article 5.1(g)(v) of the Club Constitution that as long as the Principal, Nominee or Term Membership is not terminated, that individual shall not be permitted to de-register his/her child’s Junior/Junior Term Memberships respectively, except in the event of death or where the member loses custody of the child.
• I acknowledge that if a child of a Principal, Nominee or Term Member is not registered with the Club in accordance with the Constitution Article 5.1(g)(vi), that Member will be required to register his/her child as a Junior/Junior Term Member and pay an administrative fee of $109 (inclusive of 9% GST). Failure to do either may result in disciplinary proceedings against that Member.
• I acknowledge Article 6.2(a) of the Club Constitution which states that Principal Members and Term Members shall be responsible for the conduct and debts of their spouses and children. Any misconduct or breach of a provision in the Constitution or Bye-Laws by a spouse or child of a Principal Member or Term Member shall be deemed to be the misconduct of that Principal Member or Term Member.
• I acknowledge the Bye-Laws that Sports Members, Junior/Junior Term Members (12 to 14 years) and Social Members are not permitted to bring guests.
• I agree and accept that any decision made by the Management Committee on my application shall be final.
• I hereby agree to abide by and be bound by the Constitution and Bye-Laws of the Club, as may be amended from time to time.

In compliance with the Personal Data Protection Act, we seek your consent to collect and use your personal data (e.g. name, contact numbers, mailing addresses, email addresses and photograph) as provided in this application form, or (if applicable) obtained by the Club through your membership, for the purposes of and in accordance with the Chinese Swimming Club (CSC)’s Personal Data Policy.

CSC's Personal Data Policy is outlined in the Personal Data Policy Statement, which is available on the CSC website. A hard copy of the Personal Data Policy Statement will be provided to you upon request. Our privacy policy may be updated from time to time without notice.

By signing your acknowledgement and consent below, you are deemed to have read and understood the CSC’s Personal Data Policy Statement.

I/We confirm that I/we have read and understood the CSC’s Personal Data Policy Statement and do hereby give my/our acknowledgement and consent to CSC to use my/our personal data in accordance with the CSC Personal Data Policy.

I/We hereby give consent to CSC to use my/our information to provide me/us with marketing, advertising, updates and promotion information, materials and/or documents relating to my/our membership with CSC. CSC will be able to contact me/us via all communication channels, by way of Telephone Calls, WhatsApp, Emails, SMS, Postal Letter (non-exhaustive), based on the contact details that I/we have provided without the limitation of time.

I/We am/are informed of and understand that I/we can contact CSC to opt-out of the above clauses in the future by reaching out directly to CSC.
