TRANSFER
EFFECTIVE 1 JANUARY 2007

EASY
STEPS TO EFFECT YOUR TRANSFER OF MEMBERSHIP
General Information
|
| 1. |
The transferor
(existing member) should ensure that the transferee (prospective member)
satisfies the requirements for an Ordinary/Associate (transferable) member,
and that the transferee was not a previously expelled member of the Club.
The definitions of an Ordinary & Associate (Transferable) Members
are as follows:

a) |
Ordinary
(Transferable) Members
Any Chinese Singapore citizen or Chinese Singapore permanent resident of
21 years of age and above. |
|
b) |
Associate
(Transferable) Members
Any non-Chinese Singapore citizen or non-Chinese permanent resident of 21
years of age and above. |
| 2. |
For
deceased case, the Executor(s) to ensure that the following documents are
made available to facilitate the transfer of membership: |

a)
|
Transfer
to Registered Spouse Member of the Club |
|
|
i) |
Death
Certificate |
|
b) |
Transfer
to Spouse/Child/Beneficiaries of the Deceased |
|
|
i) |
Grant
of Probate or Letter of Administration, and/or |
|
|
ii) |
Statutory
Declaration from all-lawful children and beneficiaries giving consent to
the transfer of membership to either party. (The name and identity number
of the rightful person to transfer the membership to should be clearly stated
on this document), and |
|
|
iii) |
Letter
of indemnity from all-lawful children and beneficiaries who are not taking
the membership to indemnify Chinese Swimming Club in the event of future
claims. |

| Steps
To Effect Transfer |
| 1) |
Kindly
collect a copy of the Application Form from the Recreation Complex, Reception
Counter. You can also call the Club to arrange for the form to be sent to
you. |
| 2) |
Complete
all parts of the form and call the Membership Hotline at 6885 0652/ 6 /
7 (between 9.00am - 6.00pm on weekdays) or Club's main line 6345 1221 (after
office hours) for an appointment. |
| 3) |
Transferor
and transferee are to bring the completed application form together with
the necessary documents to the Club at the appointed time. |
| 4) |
Transferor
and transferee are to sign on the application form in the presence of
a staff of the Club handling the transfer. |
| 5) |
If
the transferor is currently overseas and unable to come personally to the
Club to effect the transfer of membership, please arrange for a Notary Public
or an officer from the local embassy/consular office to certify his/her
signature on the transfer form. At the same time, an authorization letter
from the transferor stating representative's full name and NRIC number who
would effect the transfer on behalf is also required. |
| 6) |
Transferor
must bring along their NRIC, Membership Card(s), carpark Label(s), locker
key etc. |
| 7) |
Transferee
must bring along their NRIC, photocopies of +marriage certificate, +child/children's
birth certificate(s), car log card/insurance cover note (if applicable)
and 2 recent passport size colour photographs of each +family member, if
applicable. |
| 8) |
The
transferor has to settle his/her outstanding Club dues and pay the *Transfer
Fee of S$6,420 (inclusive of GST) upon submission of application. |
| 9) |
Transferee's
particulars shall be posted on the notice board for a period of not less
than 2 weeks in accordance with the Constitution. |

| Important
Notes |
| 1) |
Members
are advised to source for their own prospective transferees and agree on
the membership price before submitting their applications to the Club to
effect their transfer of memberships. Members should inform prospective
transferees in advance whether the "selling price" agreed upon
includes or excludes the *Transfer Fee of S$6,420 (inclusive of
GST). |
| 2) |
The
Club will not be involved in any dispute between the transferor and the
transferee. |
| 3) |
Application
for transfer of membership is subject to the approval of the Management
Committee. |
| 4) |
An
estimated time of between 6 to 8 weeks is required for the transfer to complete. |

Members
who joined the Club before 30 April 2000 may transfer once to a child without
paying the transfer fee. This is also provided for members who joined the
Club before 30 April 2000 and who passed away on or after 27 April 1997. |

In
the event that the Principal Member passes away, the membership can be transferred
to the registered Spouse Member without payment of the transfer fee upon
submission of the death certificate of the deceased on request. |
